Student Attendance: Report Absence or Clear Absence 
Absence Reporting for AMS: 2023-24
Thank you for using Arizona Middle School's Absence Reporting form.  Only a parent or legal guardian of the student listed on the student's emergency contacts in AERIES can fill out this form and clear an absence.  Please type your current email address below.  It should match the legal guardian/parent email registered in AERIES.

 Absences must be "cleared" using this form, emailing monica.chavez@alvordschools.org or by calling the Attendance Office (951)358-1675 within 48 hours of the absence.  

Absences that are not cleared within 48 hours may be turned into truancies. Students should monitor
attendance online and talk to their teacher if corrections need to be made.  

NOTE ABOUT EXTENDED ABSENCES DUE TO ILLNESS: A medical note must be provided when a student has had 3 or more consecutive absences due to illness. When a student has had 10 absences in a school year for illness verified by a parent/guardian, any further absences must be verified by a physician and a medical note provided to the attendance office. A student is only allowed 10 sick days in a school year. After the 10 sick days, the absence will be considered unexcused.

Reasons for unexcused absences include: being out of town, vacation, and non-school related activities.

SHORT-TERM INDEPENDENT STUDY: If your student will be absent between 3-14 days, please email nayeli.martinez@alvordschools.org to get information and determine if your child qualifies.  Students on Short-Term Independent Study have personal absences excused if they complete assignments while they are absent from school. 

We are using this form in order to support families in reporting their student's absences in an easier way, and to save staff time that can be shifted to supporting students with serious absence issues.

Please explain why your student will be absent from the class(es). According to California Education Code, the following are considered excused absences: illness, medical appointment, religious holiday, family emergency, or funeral services. Reasons for unexcused absences include: being out of town, vacation, and non-school related activities. Please note that an unexcused absence in these circumstances is different than a truancy. A student with an unexcused absence might not be excused under California Ed code but has been excused by a parent/Guardian, and they will not be subject to disciplinary action.
